Health care for the seriously ill is focused on quality care outcomes that emphasize decreased inpatient utilization and longitudinal care management with a focus on population based management. Health care chaplaincy will play a vital role in supporting clinical outcomes. All members of the health care team will be required to examine their roles to optimize quality outcomes with spiritual support for seriously ill patients increasingly be measured in terms of effectiveness and outcomes. The objectives of this talk are to describe current and future models of palliative and supportive care with a focus on population based management, to identify key metrics and drivers of quality outcomes in health care for seriously ill patients with a focus on the role of the chaplain, and to describe key roles and adaptations chaplains should prepare for in future models of care.
